To convert the raw scores you've provided to standard scores, first calculate the mean and standard deviation of the sample, then subtract each value from the mean and divide by the SD (see my work in the table below). This provides the standard score (z-score). From this you can convert the z-score to a percentage by using the z-score to percentile calculator.
